HOW
TO APPLY
-
The office of the Michigan Township Trustee is open from 8:30 a.m.
until 3:30 p.m. every Monday, Tuesday, Thursday, and Friday.
However, new applications will not be accepted after 2:45 p.m. each
day. The office is located at 2601 East Michigan Blvd. (the
former Pubco building one block east of Carroll Ave.), Michigan
City, Indiana. The office is closed on holidays
and at other times when it is necessary for the township staff to
participate in educational programs or seminars conducted by the
Indiana Township Association or other State governmental agencies. A
twenty-four hour notice will be posted prior to any office closings.
-
Any
individual or family where the head of the household is eighteen
(18) years of age or where the individual or household head is legally
and or completely emancipated at an earlier age is eligible to
apply for assistance.
-
An individual desiring assistance
should come to the township office. Anyone,
who for legitimate reasons, are unable to visit the township offices
should call in order to make other arrangements. Applicants are required to complete the application THEMSELVES. However,
the trustee will assist an applicant in completing an application if
the applicant has a mental or physical disability, has dyslexia or
cannot read or write the English language.
-
Anyone requesting assistance must first complete an "Affidavit
and Application for Relief - TA-1" to determine
eligibility. Absolutely no request will be considered without first
a completed application. In order to
determine eligibility applicants will be required to cooperate with an investigation of their
personal finances, family responsibilities, and their eligibility to
receive other types of assistance. Investigations may include a
home visit and/or contact with an applicant's relatives who may be able and
or willing to assist them. The law requires applicant's apply for and
utilize all other forms of public assistance before being granted
township assistance (IC 12-2-1-8), except in emergency situations.
Applicant's also are required to sign a "Disclosure of Personal
Information for Township Assistance" to complete an
investigation (IC 12-20-7-1).
